High Acuity Assisted Living

Ideal for: Individuals who require more comprehensive care than standard assisted living provides, including support with daily tasks such as bathing, medication management, or mobility, all in a private, discreet, and dignified setting.

  • Transfer assistance, including up to a two-person transfer
  • Medication mangement
  • Assist with personal cares including dressing, bathing, & grooming
  • 24/7 caregiver support with nurse onsite
  • 3 Restaurant-style meals daily

Transitional Care

Ideal for: Individuals who need 24/7 skilled nursing and support with daily care in a safe, structured environment that encourages independence, dignity, and meaningful engagement.

  • Caregiver on-site 24 hours a day with nurse on-call support
  • Seamless collaboration with physicians, specialists, and hospital teams
  • 3 Restaurant-style meals daily
  • Medication management
  • Transportation to local medical appointments

Rehabilitation

Ideal for: Individuals recovering from surgery, illness, or injury who need short-term, skilled therapy and medical supervision to safely regain independence before returning home or transitioning to another level of care. Residents will also have priority access to other Eagle Crest Communities housing and care options based on availability.

  • On-site therapy teams for convenient, physical, occupational, and speech therapy
  • Caregiver on-site 24 hours a day with nurse on-call support
