About Eagle Crest Communities

Eagle Crest Communities operates ten communities across the La Crosse area. Our locations include Eagle Crest on Cass, Eagle Crest at The Willows, Hearten Houses I & II, Eagle Crest South, and Riverside Transitional Care in La Crosse; Eagle Crest North in Onalaska; and Heights Townhomes, Eagle Crest at The Heights, and Hearten Houses III in Holmen.

Eagle Crest Communities is a nonprofit organization that has served the La Crosse area for over 75 years, focused on providing quality senior living and care rather than generating profit.

Eagle Crest Communities is rooted in Christian faith and guided by a commitment to caring for each other and serving the community. Our mission is lived out every day through teamwork, respect, integrity, love, and commitment in everything we do.

Eagle Crest Communities are located throughout the La Crosse area of Wisconsin, including La Crosse, Onalaska, and Holmen.

Eagle Crest Communities offers a full continuum of care, including independent living, assisted living, memory care, high-acuity care, and transitional care. We also provide short-term stays and rehabilitation services to support recovery and changing needs.

Touring Eagle Crest Communities is a great way to get a real feel for our homes, our team, and the life we offer here. While walk-ins are always welcome, we recommend scheduling your visit in advance so we can make sure someone is ready to give you our full attention and answer all your questions.

During your visit, we’ll walk you through our spaces, discuss availability, introduce you to some of our staff, and take the time to understand what matters most to you and your family. There’s no pressure — just an open, honest conversation to help you figure out if Eagle Crest is the right fit.

Absolutely! We encourage family members and loved ones to join you on your tour so everyone can ask questions, explore the community, and feel confident in the decision-making process.

The steps vary depending on the lifestyle option you choose. In general, we’ll help you reserve an available apartment and collect a deposit to secure your spot. For care-based services, a health assessment is completed prior to move-in, which helps determine approval and ensures we can meet your care needs.

Lifestyle Options & Specialized Services

It’s a process we walk through together! During your initial contact, we get a general sense of your needs and wants — for yourself or a loved one. We continue that conversation during your tour and provide materials to help guide the decision.

For lifestyle options that include nursing and care services, a nurse assessment is also required, which includes a review of medical records and other relevant health information to ensure we can provide the right level of support.

This is a question we hear often, and the answer depends on the level of care your spouse requires. In some cases, we have larger spaces where you can live together comfortably.

We also offer communities like Eagle Crest South, which features multiple levels of care under one roof — meaning you and your spouse can have separate spaces tailored to each of your needs, while still spending your days together in the same building. You stay close, just with the right support in place for each of you. Reach out to our team and we’ll help find the best arrangement for your situation.

If your care needs change over time, our team will work with you to adjust your support within Eagle Crest Communities whenever possible. Because we offer a continuum of care, residents can often transition to a higher level of care without leaving the community, helping maintain familiar surroundings and relationships. When availability allows, we prioritize current residents to support those transitions as needs evolve.

A continuum of care means residents can access different levels of support within Eagle Crest Communities as their needs change over time, without necessarily needing to move to a new community. While we cannot guarantee availability at every level, we do prioritize current residents whenever possible.

Yes! Our High Acuity Assisted Living, Memory Care, and Transitional Care communities provide 24-hour nursing services — so skilled support is always there when you need it.

All of our Eagle Crest Communities hold a Dementia Specific Certification through LeadingAge, reflecting our commitment to providing the highest quality dementia care services. Our teams embrace a holistic philosophy that fosters individualized, dementia-centered care and support — honoring each person’s unique story, needs, and dignity.

End-of-life care focuses on providing comfort, dignity, and support for individuals in the final stages of life — as well as their families. It prioritizes quality of life, pain management, and emotional and spiritual well-being during this important time.

All of our Eagle Crest Communities offer specialized end-of-life care for both current and new residents. We partner with outside hospice providers to bring these services directly to our residents, so they can remain in the comfort and familiarity of their home here with us.

Yes! We offer furnished Independent Living and Assisted Living apartments for those recovering after a hospital stay or wanting to try out our community, with support services available in Assisted Living for those who need a little extra help. Riverside Transitional Care also provides short-term rehabilitation services to help you recover and get back on your feet.

Most Eagle Crest locations have dedicated Life Enrichment staff who plan a wide range of activities and programs that support engagement, wellness, and social connection. Residents enjoy year-round opportunities, including outings, social events, live entertainment, games, and seasonal celebrations.

Eagle Crest communities offer a wide range of amenities designed to support comfort, wellness, and enjoyment, including libraries, fitness centers, movie theaters, and inviting outdoor spaces for residents to relax and connect.

Yes, meals are available at Eagle Crest Communities, with options varying by lifestyle and care level. In many care levels, meals are included in the monthly rate, while Independent Living residents have the option to purchase meals. Dining is offered in a welcoming, restaurant-style setting with a variety of nutritious menu choices.

Yes, residents are encouraged to make their living spaces feel like home. Personal touches such as furniture, décor, and keepsakes are welcome, helping each apartment reflect individual style and comfort.

Best version:
Yes, transportation is available at Eagle Crest Communities. Residents in care levels receive transportation for scheduled medical appointments, and we also offer a community bus for outings and group activities. Independent Living residents coordinate their own transportation for medical appointments.

Most Eagle Crest communities are pet-free. The exception is Heights Townhomes in Holmen, which allows one small dog up to 20 lbs. While pets are not permitted in other communities, we do welcome regular visits from pet therapy animals.

Service animals are permitted in accordance with applicable laws.
